Acute appendicitis is an inflammation of the vermiform appendix that occurs suddenly due to blockage of the appendix lumen. Acute abdominal discomfort often begins in the mid-abdomen and progresses to the right lower quadrant The exact origin of acute appendicitis is unknown, it is likely complex; luminal blockage, nutritional variables, and genetic factors are all potential causes.

  • Anorexia
  • Nausea/vomiting
  • Fever (40% of patients)
  • Diarrhea
  • Generalize malaise
  • Urinary frequency or urgency

Dr. Alfredo Alvarado established the Alvarado Score in 1986 to increase the accuracy of identifying acute appendicitis. It is a straightforward and quick evaluation system that incorporates clinical indications, symptoms, and test data.

The scoring scale includes six clinical items, each with an additive point score of up to 10 points:

  • 3 signs (Abdominal pain, Rebound tenderness, Tenderness in the right iliac fossa )
  • 3 symptoms (Anorexia, Nausea or vomiting, Fever of 37.3 °C or more)
  • 2 laboratory measures (Leukocytosis, Neutrophilia)

Scoring schema for Alvarado Score :

Each parameter is assigned a score ranging from 1 to 2, with higher scores indicating a higher likelihood of appendicitis. Negative scores are equal to 0 points. For the Positive score  except for “Right lower quadrant tenderness” and “Leukocytosis >10,000” which are equal to 2 points all other values are equal to 1 .

The maximum score is 10, with scores typically categorized into three groups: low (0-4), intermediate (5-6), and high (7-10) risk.
